About ACANY
Advance Care Alliance New York (ACANY) is a 501(c)(3) Care Coordination organization designated by the NYS Department of Health to offer comprehensive care coordination to people with intellectual and developmental disabilities. A large and diverse team of ACANY Care Managers support over 25,000 people in leading active, healthy and fulfilling lives throughout New York City, Long Island and the lower Hudson Valley regions.
ACANY was created in 2018 by 100+ diverse OPWDD community-based agencies with decades of experience providing care for people with IDD and their families. ACANY’s founding agencies and their leadership teams are pioneers in the field, advocating for community integration and against institutionalization, developing decades of innovative programs and approaches to supporting people with IDD in the community.
CCOs and the Care Manager
The NYS Care Coordination Organization (CCO) offers Care Management, health and wellness services, transitional care, individual and family supports, and community and social supports to those with intellectual and developmental disabilities (IDD). The Care Manager creates a life plan to identify interests, strengths, needs, and goals of the person with IDD. A life plan helps to eliminate barriers and create better opportunities for a healthier and more meaningful life for individuals with IDD.
The Care Manager creates a life plan working directly with the person they serve, guiding choices and coordinating services. Each life plan is personalized, reflecting a comprehensive approach to needs, wants, and preferences. The life plan is regularly reviewed by the individual and their Care Manager.
